ORBIT 360 BOOTH TERMS & CONDITIONS
No Jumping. Feet must remain still at all times
Remain on the 360 until it stops spinning completely.
Feet must remain on the platform at all times, not over the edge
Intoxicated persons may be denied.
Personal Injury Liability Waiver
It is my desire to utilize the ORBIT 360 Video Booth by EventPro Event Services, (hereafter called “360 Booth”).
I acknowledge that there could be a risk of personal injury in the use of the 360 booth.
In consideration of EventPro Event Services allowing users to be on and to utilize said 360 booth, I agree to assume the risk of personal injury, and agree to hold harmless the EventPro Event Services, its’ employees, and owners of the business, individually, from any and all liability claims, damages, losses, or cause of action, which may be incurred by the user while he/she is on said 360 booth, or while using said equipment, and which other instance may arise from any reason whatsoever, specifically including, but not limited to, any negligence on the part of the owners, its employees, or EventPro Event Services, individually, losses arising from theft or for any other reason whatsoever.
In addition, (if the user is a minor) I hereby grant permission for the user to be on the said 360 booth and to utilize the facilities and equipment thereon.
EventPro Event Services reserves the right to refuse any patron at any time for any reason.
Image Release
You agree to be photographed and/or filmed for the purposes of participating in the Orbit 360 Video Booth.
You agree that you wish to participate in the photography/filming project detailed herein and I hereby consent to the capture of my image in
photographs/footage by EventPro Event Services. I understand that my participation in utilizing the Orbith 360 Video Booth
is voluntary and that I may withdraw my consent by not participating. I further understand that, subsequently, EventPro Event Services may process (or authorize the
processing of) photographs and recorded
footage for marketing purposes. You agree that EventPro Event Services holds the license to the image/film captured during your participation.
I confirm that I am aged 16 or over and I am the legal guardian of a child under this age that I agree to the terms on their behalf.
